(some of these may be out of the scope you're looking for, but here's what I could think of)
-Being awake / aware during an operation. AHHH.
-Being buried alive.
-Being cremated while still alive. Double AHHHH. (damn you, Scrooged!)
-Something crawling *under* someone's skin (I'm glaring at you, X-Files).
